1. Locate the nearest active Duare Sarkar camp in your local ward or gram panchayat.
2. Visit the registration desk and request the official **Lakshmir Bhandar Application Form** (provided free of charge). Do not use photocopied forms from private local cyber cafes, as each official form contains a unique, stamped tracking barcode.
3. Fill out the form using blue or black ink. Complete all sections: Swasthya Sathi card, Aadhaar, Caste status, and Bank account details.
4. Attach self-attested photocopies of all required verification documents (Aadhaar, Swasthya Sathi card, Bank passbook copy, Caste certificate, and photograph).
